What consent forms did I sign?

The consent forms* that you signed were:

    • Sexual Assault Center’s Release of Confidentiality – Permission for a Sexual Assault Advocate to work as a liaison between you, Law Enforcement and the District Attorney’s Office.
    • Consent for return phone call – Permission for a Sexual Assault Advocate to follow up with you.

    *The Sexual Assault Forensic Examiner may have other forms.  The SAFE will be able to explain them and what they are for during the exam.
